Reimbursement Policy

Our practice firmly enforces our written financial policy. Every new patient is provided with the most current copy and acknowledges receipt by signature. In the event we make changes to the policy, all patients are provided an updated copy. Patients may request additional copies at any time.



Scheduling – Appointments can be made by calling our office during regular office hours or via our Follow My Health patient portal 24 hours a day.

Same Day Appointments – For those patients experiencing urgent medical problems, same day appointments are usually available. Please call the office as early in the day as possible.

Canceling – If you must cancel or reschedule your appointment, we ask that you call our office 24 hours in advance. This will allow us to use the appointment time for another patient. We do charge a $25 No-Show fee for missed appointments without prior notice.

Arrival Time- We ask that new patients arrive 15 minutes prior to your scheduled appointment to complete any required forms and paperwork. Return patients should arrive a few minutes prior to appointment time.
Late Arrival- We ask that you arrive on time for your appointment. If you anticipate arriving late for your appointment, please call our office to notify the staff. We will make every effort to work you in for your appointment if the schedule allows. However, there is a chance that we may ask you to reschedule your appointment.

After Hour Emergencies

Our answering service is available at all times when our office is closed by dialing our main number 815-932-7474. This service is available for emergencies only. Our answering service staff is trained on clinical triage and will page the on-call provider once your situation is deemed urgent. We do not refill prescriptions or birth control pills via the answering service.


We will refill prescription medications, including birth control pills only during regular business hours. Refill requests can be processed via the Follow My Health patient portal 24 hours a day. Requests via the portal over the weekend will be processed on the following business day (typically Monday unless a holiday).

Medical Records

Patients are entitled to obtain copies of their medical records upon completing a medical record release form. If the request is minimal, such as a copy of a test result, we can typically process it immediately and at no charge. If you are requesting a copy of your entire medical record please note there is a fee associated which is determined by the number of pages to be printed. Please allow 3 business days for a copy of the complete medical record.


Access to Medical Records of a Minor (Under the age of 18)

Our practice follows the federal and state laws in regards to protecting the privacy of a patient under the age of 18. As outlined by the State of Illinois, there are specific testing, diagnosis, treatment, and discussions that will not be disclosed to the parent without the written consent of the minor patient. This consent must be obtained and signed in the presence of a Westwood Obstetrics and Gynecology staff member.


Disability and FMLA Forms

Your employer’s human resource department will provide you with the proper forms for completion if you are eligible for disability or family leave following the birth of a child, or for surgery and recovery. Please complete the patient sections and bring them to our office along with instructions to where they should be sent upon completion. A $20 fee for each form must be paid prior to completion. Please allow 5 business days for form completion.
