* After  making your appointment it is important that you create an account in our Secure Patient Portal.  Once you've created a user login and password you can access any of our secure features.

* You MUST complete the Patient Registration and Health History Form PRIOR to your scheduled appointment.  By doing so, our staff will have your registration nearly complete the moment you arrive for your appointment.  Also, completing these forms in the comfort of your home allows you to access important information we may request that you would not normally carry with you to your appointment.

* Please arrive at least 10 minutes prior to your scheduled appointment time to complete registration and don't forget to bring your insurance card to every visit.

* If you have a history of abnormal pap smears, mammograms, ultrasounds or an ongoing gynecological condition, please bring any pertinent records from your previous provider.

* If you have had maternity care with another office and plan to become pregnant in the future, please bring copies of your prenatal records.

* Most importantly, visit our website often to learn about new services as we introduce them into our practice.  We strive to offer modern advances in medicine and technology to our patients.
