We continue to welcome new patients
into our practice and pride ourselves in developing long lasting
provider/patient relationships. Please take a moment to review
the suggestions below as we would like all of your visits with us to
be as pleasant and thorough as possible.

Once a new patient appointment is
made, you will receive an information packet in the mail.
Please complete the enclosed forms and mail them back to our
office so that they arrive prior to your scheduled appointment.
If possible, include a copy of the front and back of your
insurance card(s). Bring your insurance card with you to
every appointment.

It is extremely important that you
keep our office aware of any future address and/or telephone
number changes.

Please arrive at least ten minutes
prior to your appointment to finish any paperwork we may have.

If you have a
history of abnormal pap smears, mammograms, ultrasounds, etc,.
Please bring any pertinent records with you at your appointment.
