Appointment
requests can be made through our Secure Patient Portal.
Appointment requests are processed by the end of the next business
day. If you have requested an appointment and have not
received a response, please call the scheduling department at
815-932-7474.
Appointments can also be made by
calling 815-932-7474 during the following hours:
| Monday - Thursday 9am
- 5pm and Friday 9am - 4pm
Appointment schedulers are
not available between the hours of 12pm - 1pm. |
Routine
well woman exams are scheduled base on availability. We
ask that you request your appointment four months prior to the time
you are due for your exam.
Canceling an appointment -
If you need to cancel an appointment we ask that you notify our
office at least 24 hours in advance. This will allow us to
fill that time with another patient appointment.
